To apply direct just find your company and click on the "on-line application link."  You'll then be taken to that insurance company's site.  Once there you can register, get quotes, & begin applying for one of their Ohio health insurance plans.

To download an application, find your company and click on your state "paper application link."    You'll need to be able to open a .pdf document.  Then just print it out and follow the steps. (Note: Some insurance companies do not offer online application links).
